Initial Psychiatric Evaluation

(A comprehensive interview that will take approximately 60 minutes)

Establish a provider-client relationship.
Listen to your mental health concerns and symptoms.
Ask questions about:
- Your past and current mental health symptoms
- Current and history of your physical health/surgery/allergies/current and past medications.
- Current and past mental health/psychiatric hospitalization/ history of abuse and trauma/history of self-harm/history of substance use
- Family History
- Social History
- Legal History
Discuss your diagnosis.
Discuss your treatment options and medication management.
- Discuss the benefits and side effects of each medication.
- Provide you with psychoeducation.
Brief psychotherapy

Follow-up appointment

(A medication management discussion that will take approximately 10 - 30 minutes)

Assess your symptoms after starting to take the medication. Medication adjustment may be needed based on your symptoms.
Discuss any side effects from the medication you may have.
Psychoeducation regarding supplements that can help you with your symptoms.
Brief Psychotherapy
Review the treatment plan to see if any adjustments are required.

Private pay & Out-of-Network (OON) clients

Initial Psychiatric Evaluation ............................ $250

Follow-up appointment.......................................... $100

Payment methods: We accept debit, credit, HSA, FSA cards, and Paypal

Full payment is due prior to service.
We will provide you with a superbill.
Clients who have out-of-network benefits may be able to submit a superbill to your insurance to get reimbursement if approved. We cannot guarantee your reimbursement.

$60 will be charged when you cancel or reschedule less than 24 hours prior to your appointment, or do not show up for your appointment.
Copays are due via autopay. We partner with billing companies to manage our practice’s billing and payment logistics.
